One name per form. Make sure to indicate all your licenses to insure proper credit hours. There will be a $50 charge for name changes and cancellations made after September 22nd.

Meals are included with the conference registration. Extra meal tickets are available above. The venue needs a count for the evening meal.

PLEASE REGISTER ONLY ONCE. DO NO SEND IN MORE THAN ONE REGISTRATION.

NO REGISTRATIONS AFTER SEPT 22nd. $50 increase at the door.
